• Government issued ID: bring your driver’s license, ID, passport, any type of photo identification with your name and date of birth legibly printed will be acceptable. If you don’t have a US government issued ID, do not worry! Bring a foreign issued ID. The document has to have your photograph, name and date of birth written in English language.
• Health insurance card: We accept various commercial and government health insurance plans. Make sure you bring your insurance card along.
• If you do not have insurance, you will NOT be turned away. Bring proof of income and we will be happy to provide you with an affordable cash price for your visit. Bank statements, paycheck stubs, income tax records from the previous fiscal year are all helpful in determining your cash price for the visit.
• Previous medical records. If you have results of previous bloodwork, vaccination records, imaging results, cancer screening results (such as colonoscopies, mammograms or PAP smears), specialist notes. All of this information will be useful in determining a future plan of action regarding your chronic conditions as well as providing guidance for appropriate screenings and preventative measures.
• Bring your medications. Medication names are often difficult to remember, and dosages can be confusing. To avoid mistakes and gaps in your care, bring all of your current prescription and over the counter medications that you take. Our staff will review your medications and enter them in the electronic system for management by your provider.
