FEES

• At Miami Beach Community Health Center, we believe that every Miami-Dade County resident is entitled to affordable health care.
• Miami Beach Community Health Center uses a sliding –fee scale based on proof of income and individual patient’s ability to pay. We can assist you in applying for Medicaid, health insurance, Florida KidCare and reassign Primary Care Providers.
• We also accept payment from third-party payers such as Commercial Insurance, Health Maintenance Organizations (HMOs), Health Insurance Marketplace plans, Medicare, Medicaid, and KidCare.

To assist you in qualifying for health benefit programs, please take the following information to the health center of your choice:

1. Picture identification:

• • Driver’s license
  • I-94 (If foreign born)
  • Passport

2. Proof of income: (any one from below)

• • Most recent income tax return
  • Last pay-check stub
  • Unemployment letter
  • A letter on your company’s letterhead that verifies your income as an employee, or a notarized letter from your employer verifying your weekly income
  • A bank statement or any other document that states your income

3. Proof of health insurance:

• • Health Insurance Marketplace Plan card
  • HMO card
  • PPO card
  • Medicaid card
  • Medicare card

Parents or legal guardians must bring their child(ren)’s birth certificate and immunization records. Parent must be present or supply a notarized letter appointing them as legal guardian.
